The conventional order should be

Body of the letter (clinical information)
Usually the
letters are writted via personal letter pads and so, the first section of
Referral From Details will already be printed as headers and in practice it
might not be routinely need to be writted manually, though for the OET exercise
purpose, it is better that one writes it!
The same
changes have been changed in the article itself and if kindly make a note of
this change!
